Website in collaboration with NYS Department of Health, Advance Care Alliance NY, LIFEPlan CCO, and Person Centered Services CCO
ALBANY, N.Y.–(BUSINESS WIRE)–A new online directory, Disability Services NY, is now live at disabilityservicesny.org, offering individuals with disabilities and their families a centralized resource to find trusted medical providers and community supports throughout New York State.
DSNY was developed through a collaboration between the New York State Department of Health (NYS DOH), and three New York State Care Coordination Organizations – ACANY, LIFEPlan CCO NY, and Person Centered Services. Combined, the three Care Coordination Organizations cover the entire state of NY and provide Care Management services to nearly 70,000 individuals. This innovative platform simplifies the search for essential services such as day programs, employment support, healthcare, and community resources. Users can search by service type or specialty, organization name, or county, with advanced options to filter for accessibility features like parking, building navigation, telehealth availability, and staff experience with developmental disabilities.
Key Features of Disability Services NY:
- Comprehensive directory of thousands of services statewide
- Regularly updated listings with accessibility details
- Advanced search for tailored results
- Provider portal for organizations to join and update their listings
The development team approached the project with a goal to empower individuals to manage their own care as they choose – finding the right care shouldn’t be hard.
“The Disability Services NY Directory will serve as a great resource for Care Coordination Organizations, their staff, people with developmental disabilities and their families to help navigate the different community-based services that people have available to them,” said Commissioner Willow Baer, New York State Office for People With Developmental Disabilities. “It can often be confusing trying to navigate multiple service systems, as well as community-based resources, and this listing will provide people with the information they need in one place to stay up-to-date on available supports and services.”
Providers interested in joining the network can click ‘For Providers’ on the homepage. Users can also share feedback through the Suggestion Box to help improve the directory.
